Cytohistopathologic comparative study of aspiration biopsy cytology from various sites.

A statistical analysis of the diagnostic value for 244 aspiration biopsy cytology(ABC) among a total 1,043 cases from various sites was performed. ABC, using diagnostic terminology similar to that of a surgical pathology reports, was compared to the final tissue diagnosis. For the entire series, a sensitivity of 91.8%, a specificity of 99.3%, a positive predictive value of 98.9%, a negative predictive value of 94.8%, and an efficacy of the test of 96.3% were shown. There were 8 false negative and 1 false positive diagnosis. The diagnostic accuracy was 89.8%. Those results indicate that the ABC is a considerably highly accurate procedure that should be routinely employed.

Adenoacanthoma of the stomach is a rare lesion of obscure origin. Approximately 43 anthenticated cases of primary adenoacanthoma of the stomach have been compiled since the first description made by Rolleston and Trevor in 1905. No special clinical features could be found which help to separate this unusual cell type from the usual adenocarcinoma. Three theories concerning the origin of these unusual tumors are 1) squamous cell metaplasia. 2)heterotopic islands of squamous cells, and 3) tumor cells arising from the direct stimulation of undifferentiated multipotential cells in gastric mucosa. A 37-year-old Korean male was admitted to Seoul National university Hospital because of rapid weight loss, vomiting and epigastric fullness of 3 months duration. Physical examination revealed a 47kg. weighing male, but showing essentially negative in that no abdominal masses were palpable, no lymphadenopathy was noted, and rectal examination was normal. The significant laboratory data included an upper gastrointestinal series which demonstrated an large filling defect in pyloric antrum along the greater curvature, and findings of pyloric stenosis. The resected stomach contained a polypoid, focally ulcerating neoplastic mass measuring 4 cm. in maximum cross, which is infiltrating deep into muscularis. Serosa is partly involved grossly by neoplastic tissue, and several swollen lymph nudes were encountered along the greater curvature. Microscopic examination revealed infiltrating tumor masses that are comprised partly by differentiated, focally pearl forming squamous cell nests invading into external muscularis. In areas there are atypical glandular component, with evidence of mucin production.
A Histopathologic Study on 189 Cystic Tumors of the Ovary

A histopathologic study was made on 189 cystic tumors of the ovary that were obtained during a period of twelve years, from 1957 to 1968, at the Department of Pathology, College of Medicine, Seoul National University. The 189 cystic tumors of the ovary consist of 165 benign and 24 malignant tumors. The 165 benign tumors were classified histologically into simple cystoma (10.1%), serous cystadenoma (20.1%), mucinous cystadenoma (16.9%), and benign cystic teratoma (40.2%). And the 24 malignant tumors were also divided into serous.(5.0%) and mucinous cystadenocarcinomas (6.9%). The age distribution of these benign and malignant cystic tumors showed the highest incidence in the period of reproductive life, being found 83.7% between the years of 20∼50, with the average of women in benign cystic tumors occurred of 34.6 years and 43.1 years in malignant. The tumors occurred bilaterally in 11.0%. The side predilection was not observed. The size of cystic tumors varies considerably. The majority of the tumors measures 6 to 15 cm. in diameter. The smallest was a papillary serous cystadenoma of 1.5 cm. in diameter, and the largest one was a mucinous cystadenoma of 32 cm. in diameter. The relationship of papillation and locularity in serous and mucinous varieties of the cystic tumors was discussed. Among 68 benign cystic teratomas, ectodermal elements were found in all, mesodermal elements in 79.4% and endodermal ones in 41.2% of the cases.
